When should you see a rheumatologist?

Rheumatology is the specialty concerned with diseases and functional disorders of the musculoskeletal system, which includes the bones, joints, intervertebral discs, ligaments, muscles, tendons, tendon sheaths and bursae. Rheumatology deals with recognising and treating, without surgery, degenerative, inflammatory and metabolic diseases of these structures, as well as autoimmune conditions affecting the musculoskeletal system.

A rheumatology examination may be advisable, among other things:

  • for neck, back or lower back pain, or degenerative changes of the spine;
  • for persistent or recurring pain in the hip, knee, shoulder, hand or other joints;
  • for joint swelling, stiffness or restricted movement;
  • for joint cracking or gradually developing deformity;
  • for pain and inflammation originating from muscles, tendons, tendon sheaths or bursae;
  • for recurring gout or musculoskeletal effects of other metabolic diseases;
  • for the assessment and treatment of osteoporosis and its complications;
  • for suspected or diagnosed rheumatoid arthritis, psoriatic arthritis or other autoimmune musculoskeletal disease, and their long-term care;
  • for numbness or muscle weakness accompanying persistent musculoskeletal symptoms.

Musculoskeletal pain may be sudden and acute or long-standing and chronic. Some inflammatory or autoimmune diseases may be accompanied by general symptoms such as fatigue, a raised temperature or skin symptoms, and may also affect other organs. It is important to know that not all pain felt in the musculoskeletal area is rheumatological in origin. For sudden, severe chest, abdominal or neurological symptoms, you may need urgent medical assessment rather than a scheduled rheumatology appointment.

Our specialists

  • Dr. Kormos Zsófia Dr. Kormos Zsófia Specialist rheumatologist Profile

What happens during a rheumatology examination?

During the consultation, the rheumatologist reviews your symptoms in detail, how they started and changed, previous illnesses, injuries and operations, and your current medication and previous treatments. Useful information includes whether the pain is worse with movement or at rest, whether you have morning joint stiffness, swelling, numbness or muscle weakness, and how far the symptoms limit your everyday movement.

During the physical examination, the specialist examines the affected joints and sections of the spine, assesses range of motion, any swelling, tenderness and deformities and, where needed, muscle strength and certain functional movements. Laboratory tests or imaging may be needed to refine the diagnosis. Targeted joint and spine X-rays, joint ultrasound and laboratory tests are also available at Tagore Medical Center.

Based on the results, the specialist draws up a personalised treatment plan. Treatment may include medication, lifestyle and exercise advice, injection or infusion therapy and, where needed, involvement of another specialty.

How can rheumatology care help?

Rheumatology examination

Degenerative joint and spine conditions

Degenerative changes in the joint cartilage and the structures of the spine often cause pain, restricted movement and reduced load tolerance. Symptoms most commonly affect the knees, hips, hand joints and various sections of the spine. Based on symptoms and the physical examination, X-ray or ultrasound can help assess the condition more precisely where needed.

Treatment aims to reduce pain, preserve mobility as far as possible and support everyday activity. It may include medication, appropriate exercise and lifestyle advice and, where indicated, joint or soft tissue injections. For certain degenerative joint conditions, hyaluronic acid injections may also be considered. The specialist decides whether treatment is needed, and which type, based on the condition of the joint and your individual circumstances.

Inflammatory and autoimmune rheumatic diseases

Joint inflammation can have several causes. Persistent joint swelling, pain even at rest, prolonged morning stiffness or several joints being affected at once may suggest an inflammatory rheumatic disease. Rheumatology covers the assessment and long-term care of rheumatoid arthritis, psoriatic arthritis and certain autoimmune inflammatory diseases of the spine and sacroiliac joints, among others.

In addition to the physical examination, laboratory tests and imaging may be needed to make a diagnosis. Appropriate treatment of inflammatory diseases recognised in time is important to prevent joint damage and long-term loss of function. The type of medication and follow-up is determined by the type and activity of the disease and your individual condition.

Soft tissue musculoskeletal problems

Musculoskeletal pain does not always come from the joint itself. Symptoms are often caused by overuse or inflammation of muscles, tendons, tendon sheaths, bursae or the areas where muscles and tendons attach. Such problems may occur around the shoulder, elbow, hip, knee or ankle, among others. Typical features include well-localised pain that worsens with movement, and tenderness.

Where needed, the physical examination may be supplemented by ultrasound. In addition to adjusting your activity levels, medication and appropriate exercise, a targeted steroid injection into the affected soft tissue may be used in some cases.

Osteoporosis and gout

In osteoporosis, bone mass and structural strength decrease, increasing the risk of fractures. The disease may cause no symptoms for a long time, so recognising risk factors and appropriate assessment are important. In established osteoporosis, rheumatology care may include setting up and monitoring medication, as well as lifestyle and exercise advice aimed at reducing the risk of fractures.

Gout is a metabolic disease in which uric acid crystals can be deposited in the joints, causing sudden, extremely painful joint inflammation. Treatment aims to reduce acute inflammation and, in recurrent gout, to prevent further attacks and complications.

Rheumatology treatments

Rheumatology treatment is always tailored to the cause of the symptoms and the confirmed condition. Based on the specialist examination, the following treatments, among others, are available at Tagore Medical Center:

  • steroid injection into a joint or beside the spine;
  • targeted steroid injection into a muscle, tendon or other soft tissue;
  • hyaluronic acid injection into a joint;
  • pain-relief infusion therapy;
  • further injection treatments suitable for certain joint problems.

The decision to use injection treatments is always based on a rheumatology specialist examination. Not every pain or joint problem can be treated with the same method, so a diagnosis is needed first to choose the right therapy.

Frequently asked questions about the rheumatology examination

What is the difference between rheumatology and orthopaedics?

Both specialties deal with musculoskeletal conditions, but with different approaches. Rheumatology mainly provides non-surgical assessment and treatment of musculoskeletal diseases, including degenerative, inflammatory, autoimmune and metabolic conditions. Orthopaedics mainly deals with structural and developmental abnormalities of the musculoskeletal system, including those that may require surgery. There is, however, considerable overlap between the two, particularly in the treatment of degenerative joint conditions. If you are unsure which clinic to choose, please contact our patient services team on +36 20 280 3939.

What tests might the rheumatologist request?

Depending on your symptoms, blood tests, X-ray, joint ultrasound or other imaging may be needed. The specialist chooses the appropriate method based on the physical examination and previous results.

Is an X-ray needed for every joint pain?

No. The rheumatologist decides which test is needed based on the nature of the symptoms, the physical examination and previous results. For certain conditions, ultrasound or other imaging may be more informative.

What results should I bring with me?

It is worth bringing all previous X-ray, ultrasound, CT or MRI reports, lab results and discharge summaries related to your musculoskeletal symptoms, as well as a list of the medications you currently take. If you have previously had joint injections, infusions or other rheumatology treatment, please also bring the related documentation.

Can I have an injection at my first appointment?

The specialist decides whether an injection is needed based on the examination. If the indication is clear and there is nothing that would rule out the treatment or require further assessment, some treatments can be carried out alongside the specialist examination.

How long is the follow-up period?

Under Tagore Medical Center’s current terms, a repeat examination within 3 months of a rheumatology specialist examination carried out at the clinic can be booked as a follow-up examination.

Are online rheumatology consultations available?

Yes, online rheumatology consultations are currently available. However, for new or unexplained musculoskeletal symptoms, an in-person specialist examination may be needed because of the physical examination.

Do I need a referral?

You can attend our private clinic with a pre-booked appointment, without a referral. If another specialist recommended the rheumatology examination, it is worth bringing the related results and medical documents.

How much does a rheumatology examination or treatment cost?

Current fees for rheumatology examinations, diagnostics and treatments can be found in our rheumatology price list. The basic examination, imaging, injection treatments and infusion therapy may be charged as separate services.
